Roach feces observed in seams and crevices of food equipment throughout. Management instructed to initiate immediate corrective action. 7-01 (L): Mouse infestation: Mouse infestation is present. Dead mice on glue traps in basement and rodent droppings on floor edges behind equipment, on food storage equipment, on counter under toaster, and on pizza prep counter at time of inspection. Management instructed to initiate immediate corrective action. 8-06.1.1 (A): Liquid waste not disposed of properly: Liquid waste is not disposed of properly. Observed in bucket under chair near rear yard door. 8-06.1.1 (C): Drain line needs repair: Drain line is in need of repair. Repair at non-NSF 3 compartment sink, leaking at time of inspection. Management instructed to initiate immediate corrective action. 9-02.1: Employee hands not washed before food handling: Employee hands are not washed prior to food handling activities. At time of inspection no hand washing observed by employees. Hands should be washed prior to putting on gloves and after gloves are removed.
