Certified person had sink filled up for sanitizing in middle compartment and washing in last.
Notes	12-01.5.E (4) Toilet room fixtures are not in good repair. Toilet in basement male employee states is inoperable. Pad lock on door. Remove all fixtures as long as fixtures remain toilet room must be maintained in good repair.
